Instructions:

- Fill-up your details carefully and do a double check before submitting the form. (अपनी जानकारी सावधानीपूर्वक भरें और फॉर्म जमा करने से पहले दोबारा जाँच करें।)
- For Semester III students, mention changed subject name of GE & SEC (contact concerned HoD for subject details). (सेमेस्टर-III छात्रों को GE एवं SEC विषय का बदला हुआ नाम उल्लेख करना होगा (विवरण हेतु संबंधित विभागाध्यक्ष से संपर्क करें)।)
- For subject details, please visit the Sido Kanhu Murmu University, Dumka website. (विषय विवरण के लिए सिदो कान्हू मुर्मू विश्वविद्यालय, दुमका की वेबसाइट देखें।)
- Doudle-check your details before proceeding to payment gateway. Note down the Order ID for future reference or in case of disputed transaction. (भुगतान गेटवे पर जाने से पहले अपनी जानकारी दोबारा जाँचें। ऑर्डर आईडी नोट कर लें।)
- Download and take print of acknowledgement for future reference. (प्राप्ति-पत्र (Acknowledgement) डाउनलोड करें और प्रिंट निकालें।)
